The aim of our 'Home from Hospital' service is to ensure your smooth transition from hospital to home.
The service is designed to 'dovetail' with any pre- arranged help and or community care services.
Our fully equipped wheelchair, friendly transport will be able to collect you at a pre-arranged time and you will be accompanied by a carer who will be particularly sensitive to your needs.
Our carer will ensure that you are settled comfortably back home and if necessary will do some shopping and ensure your are comfortable and safe back in your own home.
You can either hire or purchase from us any equipment you may need.
